Believe it or not, it's not only possible but available (albeit with a slightly bulky antenna).<br /><br /><img src="http://www.pocketpcthoughts.com/images/web/2003/parekh-20041115-FreeTalker.jpg" /><br /><br /><i>"Freetalker is a watch walkie talkie. Now you can speak, as in films of sciences fiction, with your watch. Freetalker have a microphone, an integrated loudspeaker, a plug to connect the headset provided and 5 buttons of functions. The kit Freetalker includes/understands two watches, two chargers, two kit free hand, two batteries ion-lithium 3,7V and a complete user guide."</i>

I've seen them in the SkyMall magizines for years. Always wanted to get some (but they had the price at $40 a piece), one day I was at a store and they happened to have two left and were selling them both for $30. I must say, they work pretty well. Albeit a bit bulky, definatly not an everyday watch, but it has it's purposes.
