If you're running Pocket Informant, there is a built-in Journal feature which works quite well. Just tap n hold on the contact's name in the contact list and it should come up as an option.

Of course, that solution would only be good on your PPC, which is why WebIS (the makers of PI) also came out with Journal Sync. This syncs all the info in your PI Journal to Outlook Journal.
