can anyone help me out here? Two workers picked it up last week and didn't print out the rebate form. The rebate was good until 5/14. If someone has it or knows where to find a copy, I would really appreciate it!!

You can email Amazon and they will send you the link to it. I had this problem before and they were very helpful....
