Have you looked at the ICQ Firewall and ICQphone FAQs?

One alternative package to look at is
MS Portrait for PocketPC to PocketPC/Desktop audio conferencing. My brother and I talk once or twice a week, wireless iPAQ 3650 (his) to wireless iPAQ 3835 (mine). He uses a Linksys wireless card of some type and I use a D-Link DCF-650W. We connect wireless-ly to our respective home LANs and subsequently to the public internet over 56K dial links. Audio (half-duplex) is very good and it is certainly cool to walk around the house talking to him via my iPAQ. My wife simply rolls her eyes when I walk by...
Note that I do this through a XP Pro Internet Connection Firewall (ICF) by forwarding
TCP Port 8100 (Listen),
UDP Port 8110 (Video) and
UDP Port 8120 (Audio) to the IP of my iPAQ. I use a static IP for my iPAQ on my home LAN.
