Well, there is a cost associated with everything. If it's not the hardware cost it would be the testing cost. I can't imagine that the overall impact on the price of the phone is that big a difference.
As for selling unlocking phones, it's not that as easy as it may seem. They need to spend a fair amount of money to legally import those devices. Especially if they are not being made available for resale in a given country by the manufacturer. They can import them illegally, but then you're going to run into problems with service. :s
