It's fairly simple if you're familiar with style formatting:
1. Create chapter headers, and use the 'Header 1' style on all of them - you can either select it from the dropdown box on the formatting toolbar, or use ->Format->Syles and Formatting...
2. Create a Table of Contents from ->Insert->Reference->Indes and Tables...->Table of Contents tab - use the default options and press ok.
3. Run the word add-in - you're done!
