Stitcher Version 2.3 for iPhone/iPod Touch
"Version 2.3 of Stitcher, an iPhone app for "stitching" together favorite audio content into on-demand "stations," this week added location-based content, allowing users to create custom stations with local shows." 
This is US only unfortunately, but it is a free app. They look for the five biggest cities in each state and add their shows, and with this update they have also added subscription-based premium content as well as an alarm clock. They don't have a ton of choices but they do have something for everyone.
