Go to Start/Settings/Today and check the box next to "Use this picture as the background", and the Browse button will be come active. Then you can browse for the picture you would like to use.

Or, if you use the built-in Pictures & Videos application, you can choose "Set as Today Background..." from the right Menu softkey, which will give you additional options for setting the image's transparency (how light or dark it appears on the Today screen).

This is how to do it on a WM5 or WM6 device, but the procedure is similar going back to, I think, about 2003 devices.
