I (finally) have some comments on this problem. I just ran into it on my replacement MPx220 with the .342 firmware. :-( So obviously it has not been fixed.

And I know exactly how to re-create it or when this problem happens. It seems that the Bluetooth module has trouble running multiple sessions. I had a Bluetooth Activesync session running and I got a call. I turned on the BT headset, as you described, I could hear but the person at the other end could not hear me. Then my BT headset lost connection. I tried to answer the call, dialed the number but again could not talk on the handset or the headset. That's when it struck me that the BT layer must have crashed (BT icon was visible, but it is not a true indicator of BT's On state). So I restarted the BT and things were back to normal.
