It depends on whether or not you are looking to encode each chapter as a separate document or would like to be able to use the "tours" (exposed in the Reader as "skip by" in the riffle control) feature in Reader. If you want to be able to use this you will need to use a tool like Readworks Standard/Publisher. If you just want to be able to create an inline table of contents with the Word Add-in then the steps above that were provided by Nurhisham should be just fine.
