I'm not sure about your first query, but the second is symptomatic of icon caching with too many programs installed. You could be patient and just wait for the OS to finish loading the icons (and programs installed on external cards will slow the process down further). Alternatively, group your icons together into folders inside the Start Menu folder. This will make the display run quicker in the Programs screen. To do this just open file explorer, and go to the /Windows/Start Menu/Programs folder, and manually move around the shortcuts to the way you want them.
